You need excellent mental health services. The best you can find. That is why we are working towards accepting most insurance providers.

Please contact us ahead of scheduling your appointment to discuss medical benefits and options with you. We can also discuss what to expect when you come for an appointment to prepare ahead of time. We are here to help.

Insurance and Fees:
Please verify that the provider you would like to visit at the Cedar Oaks Clinic is in-network with your mental/behavioral health benefits. Many companies have one insurance for medical benefits and separate insurance for mental/behavioral health benefits. If we are not in-network with your insurance, you will be responsible for the entire cost of the visit. We will attempt to verify coverage for you, but you should not consider this a guarantee of coverage. Please note that we are no longer accepting Medicaid patients.

We recommend that you follow the guide below to verify your Insurance with your provider prior to your appointment.

Insurance Coverage Check - Guidance

Please call the Customer Service telephone number on the back of your insurance card prior to your first visit or when you change insurance. When calling your insurance company, ask the following questions to help determine your coverage:

  1. Is Cedar Oaks Clinic PLLC (NPI number 1184209470) currently an in-network provider for my plan? Is the provider I will see in-network for my plan?
  2. If not, what are my out of network benefits?
  3. Does my plan cover outpatient Mental Health services including E&M, Psychotherapy, counseling, Psychology assessments etc as anticipated.
  4. Do I have any mental health visits covered under the preventative care portion of my plan, (ICD-10 code z71.3)?
  5. Following the COVID-19 pandemic, do I currently have coverage for services provided via telehealth?
  6. Do I have a deductible for Mental Health Services?
  7. If yes, how much is it and how much has been met so far?
  8. Is there a copayment for each visit or do I have co-insurance? If I have co-insurance, what is the percentage of coverage?
  9. How many sessions are covered per year?
  10. What month does the policy year renew?
  11. Are there any restrictions and/or limitations to my coverage?
  12. Write down and keep the reference number for your call.

We have providers that are in-network / credentialed for those listed below. Always call ahead to confirm that your provider of choice is covered by your insurance plan. We do have private pay options – see below for current fees.

Cash Paying Patients

Private Pay Rates for Outpatient and Same Day Service
New Patient Evaluation Adult $200 / Child $250 
Follow-Up (30 mins) $150
Follow-Up (15 mins) $100
Counseling: $125/hour
Recreational Therapy: $125/hour
Health Coach / Functional Training: $125/Hour

Psychologist Services $175/Hour


Miscellaneous Fees

  • FMLA Paperwork - $75
  • Disability Paperwork - $150/packet
  • Miscellaneous School and Work Notes (excluding a basic letter to verify the patient had an appointment in the office that day) - $25/page
  • Copy of Genetic Testing/labs - $10
  • Copy of Records - $.75 per page for the first 25 pages, $.50 per page for pages 26-100, and $.25 for each page over 100 pages                    $10 minimum fee
  • Attend External School/Care Coordination Meeting/Guest Speaker or Court Attendance/Testimony - $250/hour appearances plus preparation time and/or legal advice for court matters
